How to Join BAI
1. Download Membership Application
If you would like to join Benefits Association, Inc. (BAI), simply download the PDF application below for individual membership. Once your information and joining fee is received, BAI will send you a confirmation email advising you that you can access the benefits through the online portal.*
If you are downloading this application to sign up for BAI, in conjunction with enrollment in an online dental and/or vision plan, this is not necessary. During the enrollment process of a dental and/or vision plan, you are notified, in order to join the insurance plan, you must be a member of BAI. The page also informs you, the premium quoted includes your BAI dues.
Any applications received before the 20th of the month will receive an effective date of the 1st of the following month. If your application is received after the 20th of the month, the effective date will be the following 1st of the month.
